Lakeside Healthcare is a new type of NHS general practice which builds upon and celebrates all that is great about traditional primary care services. We are local surgeries run by doctors, nurses and associated staff who know their patients and care deeply about the quality of health provided in their communities. Purpose of Role Working alongside a solid team of Clinicians and be a part of a wider team of partners across the partnership, complimented by a superb multidisciplinary team which includes: Team of nurses and healthcare assistants supporting and delivering mental health, chronic disease and treatment room services. Friendly team of 4 Partners, meaning support is always available. Long established training practice, providing opportunities for career development. Established
pharmacist team supporting medication reviews, chronic disease and clinical correspondence. Large multi-disciplinary team including Social Prescribers Link Workers and Safeguarding specialists. Dimensions Key to the role is the need to work closely with stakeholders across the business and be a trusted and credible partner to managers and the executive team, providing them with the necessary and consistent support and guidance to engage and develop their people and providing pragmatic solutions to complex issues. Responsibilities Clinical responsibilities: In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, referrals and correspondence in a timely manner. Making professional, autonomous decisions in relation to presenting problems,
whether self-referred or referred from other health care workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness. In consultation with patients and in line with current practice disease management protocols, developing care plans for health. Providing counselling and health education. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate. Recording clear and contemporaneous consultation notes to agreed standards. Collecting data for audit purposes. Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible. EPS is now implemented at the Practice). Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate. In general, the Partner will be expected to undertake all

the normal duties and responsibilities associated with a GP working within primary care. Complete care plans required for local enhanced services. Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety. A commitment to life-long learning and audit to ensure evidence-based best practice. Contributing to evaluation/audit and clinical standard setting within the organisation. Contributing to the development of computer-based patient records. Contributing to the summarising of patient records and accurate Read-Coding of patient data. Attending training and events organised by the practice or other agencies, where appropriate. Other responsibilities Flexibility and adaptability, with the ability to manage change and adapt as required and the ability to remain calm under pressure and deal effectively with conflicting priorities. A proven ability to lead IT projects, with strong
organisational skills and the ability to independently drive projects to completion, giving due attention to deadlines and quality. Initiative and a pragmatic approach, with the ability to interpret data and make sound business focused recommendations in relation to IT issues. Comfortable with being challenged and equally able to challenge the status quo, with appropriate consideration of stakeholder interests. Professionalism and diplomacy, with top quality interpersonal skills and an ability to handle matters with the appropriate level of sensitivity and empathy.
