Full Job Description

An exciting opportunity has arisen for a suitably qualified individual to join our Heart Failure Rehabilitation team. We are looking for an enthusiastic nurse already involved in cardiac care, who wishes to become more specialised in their work.

The post holder will ensure the delivery of high quality, clinically effective care by being a senior member of the Heart Failure team.

They will assist with the development and provision of the Heart Failure Rehabilitation programme that meets the needs of patients and their families, in accordance with national and local policies and initiatives., The successful applicant will work as part of a team, and on occasion, as an independent practitioner, facilitating cross boundary and inter-agency working looking after people with heart failure. This involves identifying people with heart failure as inpatients and leading them through rehabilitation as outpatients.

They will demonstrate advanced evidence-based practice and through detailed holistic, written, electronic and verbal assessment, implement appropriate advice, guidance and evaluation of patient care within own specialty.

The post holder will effectively communicate complex, sensitive or contentious information to the multidisciplinary team, gaining their co-operation. They will also to liaise with broader community services to ensure seamless care provision.

Undertaking audit will form part of their duties and inputting data on to NICOR. Opportunities to get involved in research will also be available.

£35,392 to £42,618 a year pa pro rata, Salary is dependant on experience
